FOUNDATIONS PROGRAM

Building a foundation for longevity & wellness

Healthcare that delivers long term disease prevention and lasting wellness should really begin at an early age when lifestyle choices and their effect on the body and mind can have a profound effect on future health and longevity. The Harrison Foundations Program is Nurse Practitioner-led and designed for those between the ages of 12 and 24, a crucial time for building the foundation of wellness. Understanding genetic and lifestyle-based risk factors, providing education and identifying early signs of disease are the cornerstones of this important program.

You will receive year-round care appointments with your Harrison team which includes your nurse practitioner and/or physician, a nurse, a mental health professional, a genetic counsellor, and a dedicated care coordinator.

The Foundations program includes:

  • An age-appropriate Personalized Health Assessment
  • Full physical examination & lab work
  • Fitness & functional movement assessment with an exercise physiologist
  • Nutritional assessment with a registered dietitian
  • Gold standard of prevention, early detection & health management
  • After-hours coverage, for urgent care needs
  • Comprehensive mental health assessment when indicated, care planning, and coordination with community treatment partners as required
  • Motivation for developing healthy lifestyle choices
  • Guidance for avoiding risky behaviours and ensuring healthy sexual habits
  • A complete, dedicated team of healthcare professionals
  • Navigation and advocacy services for public and private care options
  • Tight collaboration with external providers, including pharmacy and medical specialists
  • Urgent case management
  • Travel medicine consultation
  • Genetically Informed Care consultation for greater health plan precision
  • Secure, online, globally accessible personal health record (HealthChart)

THIRD PARTY COVERAGE

The Foundations program for young adults also includes third-party extended medical coverage¹:

  • Access to a curated network of international specialists for no-cost, timely specialist consultations

Cost

Fees are payable monthly or annually. Parent or guardian must be subscribed to the Premier program. Please see our Fees page for program costs.

* Note: Fees for Harrison Programs are strictly for the uninsured components of care, including non-physician care. Insured physician visits and after-hours care, prescribed tests, and any other publicly insured services will be billed to the provincial health insurance plan by Harrison or by its physicians. Provincial health insurance plans invariably provide important physician services in the fields of prevention and disease management. Subscription to a Harrison Program is not a precondition to access insured physician services.

¹ Third party coverage is not applicable to independent clients (not associated with a Premier client), and those age 21-24 must be enrolled as full-time students.
